Temazepam Among Its Relatives

Five benzodiazepines are FDA-approved specifically for insomnia. The big difference between them is how long they, and their breakdown products, stay in the body:

Temazepam sits in the middle: long enough to help people stay asleep, and without the long-acting metabolites that make flurazepam and quazepam accumulate. That’s a large part of why it’s one of the most commonly prescribed benzodiazepine hypnotics in the US, and why the American Academy of Sleep Medicine’s 2017 guideline lists it as an option for both falling asleep and staying asleep.

Why There’s a 22.5 mg Capsule

Temazepam comes as capsules, not scored tablets, so a dose can’t be split in half. The four strengths (7.5, 15, 22.5, 30 mg) exist so a prescriber can adjust in 7.5 mg steps without splitting anything:

• 7.5 mg: starting dose for older or frail adults
• 15 mg: usual adult dose
• 22.5 mg: a middle step when 15 mg isn’t enough but 30 mg is too much
• 30 mg: top of the usual adult range

Never open capsules or combine strengths to make your own dose. If your dose needs to change, ask your prescriber for a new prescription at the right strength.

Before a Procedure: What “Off-Label” Means Here

Some clinicians use temazepam before surgery, dental work, or imaging to ease anxiety. That’s an off-label use: temazepam’s only FDA-approved indication is short-term insomnia. In that setting:

The Diazepam Connection

Here’s something most temazepam pages don’t mention: temazepam is one of the substances your body makes when it breaks down diazepam (Valium). Diazepam is converted to nordiazepam and temazepam, and both are eventually converted to oxazepam.

Why that matters:

• Drug testing: someone taking prescribed diazepam can test positive for temazepam, even if they’ve never taken a temazepam capsule.
• Combined use: taking temazepam on top of diazepam means stacking the same drug family, and in part the same molecule. It’s not a normal regimen without explicit prescriber direction.

Temazepam and Drug Tests

• Standard urine drug screens usually include a benzodiazepine panel. Some common immunoassays detect temazepam and its breakdown product better than others, so results can vary by test. Confirmatory testing identifies the specific drug.
• How long it’s detectable depends on the dose, how long you’ve been taking it, your age, and the test type. Occasional use generally clears faster than regular nightly use.
• Keep your prescription information handy. A legitimate prescription is what explains a positive result to an employer or medical review officer.

Temazepam After 65

This deserves its own section because older adults are among the most frequent temazepam users and also the people it’s riskiest for.

The concern: the American Geriatrics Society’s Beers Criteria recommend avoiding benzodiazepines in older adults, citing higher sensitivity, slower clearance, and increased risk of falls, fractures, cognitive impairment, and motor-vehicle crashes. If temazepam is used, the starting dose is 7.5 mg.

The encouraging part: stopping is often more achievable than people expect. In the EMPOWER trial (JAMA Internal Medicine, 2014), older adults who received an educational brochure about benzodiazepine risks and how to taper were far more likely to stop within six months than those who didn’t: 27% vs. 5%.

How to raise it with your prescriber:

• “Is this still the right medicine for me at my age?”
• “What would a slow taper look like?”
• “What can I do for sleep instead, such as CBT-I, or a non-controlled option like Rozerem?”

At the Pharmacy Counter

A quick checklist for when you pick up temazepam:

• Strength check: does the capsule strength match what your prescriber told you? 7.5 and 30 mg capsules can look similar across generic makers.
• Other benzodiazepines: are you also filling alprazolam, lorazepam, clonazepam, or diazepam? Say so, even if a different doctor prescribed it.
• Opioids: any pain medicine, cough syrup with codeine, methadone, or buprenorphine? This is the combination the boxed warning is most concerned about.
• Refills: as a Schedule IV drug, temazepam can be refilled at most five times within six months of the original prescription.
• Storage: keep it out of reach of others, ideally locked. Benzodiazepines are frequently diverted from home medicine cabinets.
• Disposal: use a drug take-back site or DEA take-back day for leftover capsules instead of keeping “spares.”
