Usually for: opioid-tolerant patients, typically people with cancer pain or long-term severe pain who already take opioids daily, often as breakthrough doses alongside a long-acting opioid. Equivalent to: roughly 45 mg of oral morphine. Notes: this dose can cause fatal breathing problems in someone without tolerance.

Why no acetaminophen matters
