The MAOI danger

Meperidine combined with an MAO inhibitor (phenelzine, tranylcypromine, isocarboxazid, selegiline, and also linezolid and IV methylene blue) can cause a sudden, severe reaction: very high fever, rigidity, agitation, blood-pressure swings, coma, and death. It’s one of the most dangerous drug interactions in medicine.

Why is Demerol used so rarely now?

Because its metabolite, normeperidine, can cause tremors, hallucinations and seizures, oral doses are weak, and it offers no advantage over other opioids for most pain. Most clinicians now choose morphine, hydromorphone or oxycodone instead.

Is meperidine stronger than morphine?

How long does Demerol stay in your system?

Meperidine clears within about a day. Normeperidine can take several days, longer with kidney problems. Standard opiate urine screens may not detect meperidine; specific tests are needed.

Can I use Demerol during labor or while breastfeeding?

It was once common in labor but is now used less, because it and normeperidine cross the placenta and can make newborns drowsy for days. Is Demerol addictive?
